All codex entries are reset when a New Game + is started.
When The Journey to Meridian is acquired, the symbol will get locked to the Codex at the top of the screen and to the Scourge codex entry. There is no way to remove this icon. Once this happens, you will have to open the Codex screen and check to see if any of the categories are showing a new codex entry has been added.
